[fixed] Remove Module problems.

Greetings.

I just installed VB 1.33 and have found some problems with the "Remove Modules" process.

It would appear that the module list isn't dynamically generated or the data is getting corrupt. For example, I had 4 cameras in the profile and when I removed camera 1, the other cameras were renamed but the Remove menu listing didn't update to reflect this. This also happened with the Video Effect modules.

Eventually some remove menu items don't work because the names don't match the modules on the screen.

Let me know if this wasn't clear.

Ah, darn. The one thing I did not test.. It's actually worse than that: removing of a module that is in use can result in an error. This is actually intentional as I was testing something else, but forgot to change it back.

I see there are actually 2 bugs, the one you reported (wrong numbering) and the one I initially saw (error when removing camera in program/preview). Your generic topic covers both so I'll leave them here.
